Burn Injury Reporting

This form is for use by hospital and medical personnel to report burn injuries treated in their facility, per RIGL 23-28.2-24.  The Burn Injury Reporting Law states that any burn injury or wound of second or third degree burns to five percent (5%) or more of the body and/or any burns to the upper-respiratory tract or laryngeal edema due to the inhalation of super-heated air, and every case of a burn injury or wound which is likely to result in death, shall be reported at once to the Office of the State Fire Marshal.

Reports are to be made by the physician attending or treating the case or the manager, superintendent or other person in charge of the medical facility.
